Required Forms

The LFHS Consent to Treat waiver form must be completed before players can participate in a LFHS Scouts Hockey sanctioned event. Please download the waiver to the right, complete and upload to your player participant portal. After you have the form completed, please follow these steps:

NEW REGISTRANTS (who do not already have a CTT on file)

-Go to "Account" on the upper left hand side of www.scouthockey.com (make sure you're signed in)

-Click on your registrant

-Scroll down to "registration history" and select Fall 23/24 "view details"

-Scroll down to "documents" where you will see "CTT." Click on the "required to roster" link to upload your completed form. 

RETURNING REGISTRANTS (who do already have a CTT on file)

-Go to "Account" on the upper left hand side of www.scouthockey.com (make sure you're signed in)

-Click on your registrant

-Scroll down to "documents" where you will see "CTT" with the status "approved" 

-Please reject the existing doc

-You will then get an email telling you it's been rejected and providing you with a link to use to upload your new completed one.
